By signing your name electronically on the following documents and uploading any digital images, you are agreeing that your electronic signature is the legal equivalent of your manual signature and all information provided is true and accurate. Additionally, you are consenting that your information will be shared with the Medical staff at Sumter High School to be used in the event of an emergency and/or treatment.

Prior to participating on a sports team at Sumter High School, both parent and athlete must create individual SportsWare accounts. The primary purposes of the account are to document injuries/treatment, create student-athlete emergency information sheets, and collect forms required by South Carolina state laws.

Access to Sportsware online will eliminate the need for physical forms to be collected and transported reducing the risk of lost forms and possible identity theft as students carry these forms back and forth to school. This will also allow for information changes to be much more quickly and accurately.

Each year, every athlete must renew their physical and SC State required documents. It is very important that each athlete and parent utilize the specific instructions provided below to complete this process. Please provide all information that is applicable (phone numbers, addresses, etc.) Whenever this information changes, please login to update it as soon as possible.

Starting April 1^st, 2024 paper forms will no longer be accepted. Only digital submissions through Sportsware’s online portal will be valid.
